Summary
Use Intake Form Scoring to determine vendor risk based on multiple factors beyond just Data Types. This feature allows you to assign point values to intake questions and automate risk assignments based on those scores, eliminating the need for manual spreadsheet calculations.
With this feature, you can:

🚀 Getting Started
This article will show you how to:
- Enable Scoring on your Intake Form
- Assign point values to your questions
- Use Intake Score to automate Pre-Questionnaire Workflow & Logic
⚙️ Enable Scoring
- Go to Settings (top-right) and select Assess Settings from the drop-down menu
- Select Custom Intake Form tab from the left-side menu
- Toggle ON the Enable Scoring option (located at the top-right of the page)
- A confirmation window will appear with additional instructions - click to confirm
📝 Assign Point Values to Your Intake Form Questions
Once scoring is enabled, you can assign point values to questions:
Assigning Points:
- Use the Score box next to each answer to set point values
- Important: You can only assign points to custom Dropdown (single or multi-select) and Country form components
- Default fields like Business Criticality Level or Sponsoring Business Unit cannot be scored (their options are managed in Program Automation)
Tracking Score Range: The Score Range Available box (bottom right) displays the total possible score for your form:
- Maximum: Sum of all points in the form
- Minimum: Sum of points from required questions only (optional questions default to '0')
🤖 Automate Actions using Point Values
Set up automated actions based on Intake Scores:
- Go to Assess Settings > Program Automation > Define Pre-Questionnaire Workflow & Logic
- Select New Workflow
- In the Condition dropdown, select Intake Score
- Choose your scoring condition:
- Specific value: If Intake Score is [enter score value]
- Range: If Intake Score is between [x score value] and [y score value]
- Set up an Action:
- Assign Risk to: Choose Inherent Risk from dropdown
- Send the: Choose a questionnaire from the dropdown
- Send Vendor Added Email to: Enter email recipient address
- Each workflow can only contain one Intake Score condition (statements are connected by 'AND')
- To add more conditions, create a new workflow
- Data Types and Classifications logic will override Pre-Questionnaire Workflow & Logic when editing via the Vendor Details page
The Vendor Intake Score value is available in the Vendors page and in Reporting (Resources [drop-down menu] > Reporting)

Can I assign points to all intake form questions?
No, you can only assign points to custom Dropdown (single or multi-select) and Country form components. Default fields like Business Criticality Level cannot be scored.
What happens if I change Data Types after a vendor is added?
Data Types and Classifications logic will override any Inherent Risk previously assigned through Pre-Questionnaire Workflow & Logic when editing via the Vendor Details page.
Can I have multiple Intake Score conditions in one workflow?
No, each workflow can only contain one Intake Score condition since all statements are connected by 'AND'. Create separate workflows for additional conditions.
Where can I view the Vendor Intake Score?

How is the minimum score calculated?
