Summary

This article will give you the basic tools you need to familiarize yourself with Whistic's Questionnaires.

Access the Questionnaires tab by selecting Assess Questionnaires at the top of the page.

📋 Industry Standard Questionnaires

Whistic provides all the industry standard questionnaires found on the Questionnaires page. Depending on your contract we will also include premium standards that require a subscription (SIG, SIG Lite, etc.).

Pro tips:

  • Use the search bar located at the top left-hand side of the page to pull up questionnaires based on name or description
  • Toggle ON the Pending Requests filter (near the search bar) to filter for any questionnaires with an outstanding request

📨 Requesting a Questionnaire

You can send a questionnaire request from the Questionnaires tab - hover over the questionnaire you are interested in (or use the search bar to filter) and select Request from Vendor option.

Requesting a questionnaire from a vendor can also be done on the Vendor Details Page under the Assessment Activity section.

  • Go to the Vendors tab on the menu select the vendor to open their Vendor Details Page Under Assessment Activity click on Start Assessment Choose the Questionnaires request type Select the Questionnaire(s) you want to request in the dropdown menu Hit Continue and Send the Request.
  • You can cancel a request or resend it to continue due from the Assessment Activity.
Pro tip: Quickly find the most current version of a questionnaire by using the Updated Versions filter next to the search bar (toggled on below).

⚙️ Custom Questionnaires

To create a Custom Questionnaire, scroll down the Questionnaires page until the Custom Questionnaire Drafts section, hit Create New Questionnaire.

You can add logic via the Custom Questionnaire Builder. The builder will also include a Manual, Use Existing (to duplicate) and AI Import, whichever is your preference.

Custom questionnaires that have been completed will show at the top of the Questionnaires page under the Custom Questionnaires section. You can self-assess against them or request these from a vendor.

If you need to edit an existing Custom Questionnaire, scroll to the bottom of the page or use the search bar to find another listing of your custom questionnaires (draft version) and edit them there.

🎯 Score Builder

Use the Score Builder to customize the weighting of each question within a questionnaire.

✅ Complete a Questionnaire

You may be asked to complete an assessment/questionnaire in Whistic which can be accessed from the link in the email or on your Dashboard.
