Summary
Once a questionnaire has been completed it can be added to your Basic Trust Center as a completed questionnaire. Please refer to the article Completing a Self-Assessment for your Whistic Trust Center for more information on how to self-assess on Industry Standard questionnaires.

Steps
1. Click on Trust Center from the top navigation menu and select Trust Centers
2. Choose a trust center or select the 3 dot (...) menu to create a new one:

3. Once the correct trust center is chosen, you will want to click 'Edit Trust Center' at the top of the screen to go into 'Edit Mode'. Once in edit mode, your screen will look like this:
4. Next, you will scroll down to the 'Questionnaires' section. Once there, you will click on '+Questionnaire' on the right-hand side:
5. Changes are auto-saved, but once you have added the questionnaire, you can select 'Done' at the top of the page to exit 'Edit Mode'
