Summary
Here is a short guide on how to conduct a review on a questionnaire. This can be a questionnaire that was sent to a vendor and returned or a questionnaire that was shared via a Trust Catalog or Whistic Profile.
Questionnaire Dates
All questionnaires in the Trust Catalog and Profiles, as well as on the Questionnaire itself, will have a Created Date and an Updated Date (if applicable).
These will also show on the questionnaire details, in addition to Requested, Returned, and Shared.
Questionnaire Reviews & Reports
When a questionnaire has been completed by one of your vendors and it is shared back:
- Admins receive an email notification
- The assessment activity for that vendor is updated on their vendor details page giving you an option to View Report.
- The questionnaire progress will also be updated in the Vendors tab
- Click View Report (view the completed questionnaire) on the vendor details page (under Assessment Activity)
- Once the page loads all responses, click Start Review at the top left:

Green Icon - compliant answer

Red Icon - non-compliant answers

Gray Icon - are answered as Not Applicable/Skipped (due to logic in custom questionnaires)

Mark as reviewed - a completed question that does not need to be flagged or require any clarification.

Flag as Issue - any issues flagged will be included in the final report along with comments if any are added.

Needs Clarification - if this option is selected the external checkbox will be checked indicating that any comments added will go back to the vendor

A box at the bottom of the screen will tally the number of needs clarification questions to be sent. Click the text to expand/collapse the box.

Click Send when you’re ready for those questions to go back to your contact at the vendor. They will get an email notification and can click that to go into the questionnaire and respond.

- Clicking Finish Review opens a dialog box:

Flagged issues and needs clarification are listed out here.

Use the Text box to write up any final comments.

A follow up reminder can be added here. Add comment, date, and assign to a member of your team.

The Assessment status can be changed.

Finally, the Create report option can be clicked to complete the review. Export the review as a PDF or Excel file if needed.
